How it works?

Kurt is a tool for creating digital forms. The types that are available are surveys and registrations. Kurt distinguishes between three different user roles - orderer, recipient and reviewer. The orderer is the person who makes the order and who is responsible for the form. A recipient is a person who must provide an answer in the form. One and the same person can have several roles in Kurt.

Order a form

Questionnaire and registration

To create a form, you first need to log in with your username and password A. Then you order a new Kurt by following a step-by-step guide where you select the type of form, create questions and specify settings for how the form should work. Opening and closing of the form is handled automatically based on the dates specified in the order. You can also create a new order based on a previous Kurt by clicking on this under My page and selecting the Copy tab etc.

Edit a form

As long as a form has not yet been opened, the customer can edit it freely. Once the form has opened, some fields will be locked for editing.

Login Protected Forms

It is possible to login to protect a form, ie. restrict so that only certain recipients can respond. This can only be done if all recipients of the form have an account at Uppsala University. In the case of a login-protected form, the recipients must log in with their username and password A in order to be able to respond.

For login-protected forms, it is possible to generate reminders for those who have not responded.

Unlocked form – without login protection

A form that is not login protected can be answered by anyone. A respondent can submit several answers. The orderer needs to ensure that the link is distributed/shared to those who are to respond.

Recipients fill in

Once the form has been opened, it is time for recipients to respond. This is done by the recipient going to the link that the orderer has shared / sent out and filling in the form. For locked forms, the link is sent out to the recipients upon opening. A recipient can always start filling out their form and then finish and resume at another time. The recipient can edit their submitted answer as long as the form is still open.

Preview

Once the form has been created, there is the option to preview. To share the preview with others, copy the URL. You can then send it to whoever you want.

Overview

By logging in to Kurt and going to My Page, clients, recipients and reviewers can get an overview of their forms. As an orderer, you can see the opening and closing dates, follow how many have responded and see summaries for the open and closed forms. It is also possible to export data for later processing, and for locked forms to send reminders.

Compilation and export of data

Received data is compiled in reports under the Summary and Statistics tab of the form. The reports are available in PDF format (a summary and a report for individual answers) and in Excel format.

Bilingual form

If you wish for a form to be bilingual (both Swedish and English), you need to formulate each question and possible question alternatives in both languages, (e.g. 1. Do you have any allergies?/Har du några allergier?). You can also create two form, one in Swedish and one in English. The language setting found under the tab About the form has nothing to do with translation of questions, but only controls the language of the framework, i.e. the text that you cannot edit and influence yourself.

GDPR

Forms in Kurt are anonymous as long as the orderer does not request information that can identify the recipient. We guarantee that Kurt follows the security classification required for the processing of personal data. Likewise, IT guarantees that the servers and are adequately protected. However, you who administrate a Kurt form must guarantee that you administrate the personal data you collect in a secure way, such as that you handle your computer and your passwords in a way that reduces the risk of personal data falling into the wrong hands. Or that the files you download from Kurt are not left to people who will not process the personal data. That you guarantee this is not really specific to Kurt, but is a responsibility you have as a government official, and is actually something that applied before the GDPR came into force. If you need help developing routines that may be appropriate in your specific case, start by discussing this with your manager. The Data Protection Officer can also assist you in developing appropriate procedures.

Question types

The following question types are available:

  • Short text question - When you expect a short text answer
  • Long text question - When you expect a longer text answer
  • One-choice question - Only one option can be choosen
  • Multiple choice question - Multiple options can be choosen
  • Scale question - Rating scale with any number of degrees
  • Group Scale Question - Multiple scale questions with the same assessment scale
  • Drop down - Only one option can be selected, one-choice question where you have many options in the list
  • Ranking - Possible to sort fixed options in a specific order
  • File Upload - When you want the recipient to upload a file (document)

For the two variants of scale questions, a mean value is automatically calculated at compilation. The scale at which the mean is calculated starts with 1 for the first value and increases by 1 for each answer option.

Glossary

Survey

A survey is the questionnaire that recipients fill out when participating in a survey. A survey is created by someone placing an order in Kurt.

Registration

A registration form can be used to find out, for example, who will be attending a meeting.

Recipient

A recipient is a person who can respond to a survey, or registration in Kurt.

Orderer

A orderer is a person who is to conduct a survey or receive notifications.

Co-orderer

Has the same administrative rights as the orderer. In other words, co-customers can edit the form and access reports.

Draft

A form that a orderer has started creating but has not finished.

Waiting to open

A form that has been ordered, but has not yet been opened to recipients.

Open

A form available for recipients to fill out.

Closed

A form that is no longer available to fill out.
